Ade Lee 00f6f14bbf Fix some ca_id related bugs, add more functional test code
Fix some bugs introduced in ca_id code that prevent cert issuance.
Added missing policy for ca_id calls.  Also added functional test code
to retrieve ca_id for cert functional tests.

Implements: blueprint identify-cas
